چکیده

Nineteen patients with obliterative arteriosclerotic disease of the legs and suffering from intermittent claudication were studied before and during a 6 months' training program. The maximal walking distance and the calf muscle blood flow during walking (xenon-133 clearance technique) was recorded at monthly intervals. A significant correlation was established between improvement in maximal walking distance and a change on exercise toward more normal (that is, more adequate) blood flow in the calf. In particular, it was found that calf muscle blood flow during walking was increased by physical training. The data suggest that functional factors (for example, better coordination of the working muscles or a decrease of in-between-step intramuscular pressure) as well as anatomic factors (increase in number or diameter of collateral vessels, or both) are involved in increasing collateral efficiency.

Exercise Physiology Efficacy of Quantified Home-Based Exercise and Supervised Exercise in Patients With Intermittent Claudication A Randomized Controlled Trial

Background—This prospective, randomized, controlled clinical trial compared changes in exercise performance and daily ambulatory activity in peripheral artery disease patients with intermittent claudication after a home-based exercise program, a supervised exercise program, and usual-care control. Methods and Results—Of the 119 patients randomized, 29 completed home-based exercise, 33 completed...

Exercise prescription using the heart of claudication pain onset in patients with intermittent claudication

OBJECTIVE To assess the acute metabolic and cardiovascular responses to walking exercise at an intensity corresponding to the heart rate of claudication pain onset and to investigate the effects of a 12-week walking training program at this intensity on walking capacity. METHODS Twenty-nine patients with intermittent claudication were randomly allocated to the walking training (n=17) or contr...

Medical treatment of peripheral arterial disease in the elderly

Smoking should be stopped and hypertension, diabetes mellitus, dyslipidemia, and hypothyroidism be treated in elderly patients with peripheral arterial disease (PAD). Statins reduce the incidence of intermittent claudication and improve exercise duration until the onset of intermittent claudication in persons with PAD and hypercholesterolemia. Antiplatelet drugs such as aspirin or clopidogrel, ...

An exercise test in intermittent claudication.

In most cases of intermittent claudication the diagnosis is apparent after the history and clinical examination have been completed. Although the history itself may be misleading, clinical examination is usually fruitful; the signs of obliterative arterial disease are present in most, and cases of anrmia and coarctation of the aorta that present with intermittent claudication should be easily d...

Walking exercise in patients with intermittent claudication. Experience in routine clinical practice.

BACKGROUND In patients with intermittent claudication, exercise in the form of walking is effective in reducing pain and maximising achievable walking distance. However, data are lacking on the implementation of walking exercise in these patients. AIMS To explore the current behaviour and views of patients with intermittent claudication towards taking walking exercise. DESIGN OF STUDY Posta...

Intermittent claudication: cost-effectiveness of revascularization versus exercise therapy.

PURPOSE To compare the costs, effectiveness, and cost-effectiveness of alternative treatment strategies for intermittent claudication. MATERIALS AND METHODS By combining data from the literature and original patient data, a Markov decision model was developed to evaluate the societal cost-effectiveness. Patients presented with previously untreated intermittent claudication, and treatment opti...
